Your dog is important to me and I will do everything in my power to protect your loved one when in my care from loss, harm or injury.
I understand and agree that in releasing my dog(s) in The Dog Walker (Adrienn Torok) care, The Dog Walker has relied upon my representation that my dog(s) is/are in good health and have not harmed or shown aggressive or threatening behaviour towards any person or any other dog.
I understand that I am solely responsible for any harm caused by my dog(s) while my dog(s) is/are attending walks, pet sitting or any activities with The Dog Walker. (Adrienn Torok) I assume all related risks, both known or unknown to me, of my participation in this service.
I further understand that due to the way that dogs interact with one another, minor cuts and scratches can occur even though the dogs are carefully supervised at all times.
While my dog(s) is/are in the care and custody of The Dog Walker (Adrienn Torok), if I am unreachable in the event of an emergency, I hereby authorize The Dog Walker (Adrienn Torok), its agents, and/or representatives to seek immediate veterinary care for my dog(s).
I agree to be financially responsible for any and all costs in connection with, veterinary, medical or other treatment. I am aware and understand that I should carry my own pet insurance.
Additionally, if my dog(s) do have any conditions that will increase the likelihood of experiencing injuries while engaging in these activities then these conditions must be made clear to The Dog Walker (Adrienn Torok).
I agree that my animal will have all vaccinations, worming and flea/tick treatments kept up to date.
When my dog(s) are staying with you I will provide their food, any on going medical treatments, beds, collars,leads, clothing, favorite treats. Any dietary needs should be discussed before their stay.
Also if the stay need to be extended you must advise The Dog Walker (Adrienn Torok) at least 48hrs before.
All animals must also have a collar and ID tag with the owners contact details and have a microchip with correct details.
Although we carefully screen all applicants, occasionally we discover that this is not an appropriate service for every dog. The Dog Walker (Adrienn Torok) reserves the right to permanently remove a dog from our services at any time. I also agree that pictures and videos of my dog(s) might be used for social media and advertising purposes.
Fees / Charges
Dogsitting: Full payment for the whole duration must be fulfilled before or on the day of the dog(s) arrival, any additional charges/fees (e.g. food,medical treatmens) must be paid within 48hrs of being requested.
Dog taxi: If your dog(s) pick up or drop off dates happen to be over the weekend there will be an additional charge for that. Monday-Friday is free of charge.
Dogwalking: Must be paid on the day before or before the walk.
Loyalty card : Full amount Must be paid before the first walk.
By signing this form, I forfeit all rights to bring a suit against ‘The Dog Walker‘ (Adrienn Torok) for any reason. In return, I will receive the right for my animal to participate in all activities conducted by The Dog Walker.
